Should I ever add more battery acid?

As far as I know, I'm only supposed to keep an eye on the battery fluid level and add distilled water when needed. But after doing this over and over for years, doesn't there come a point when I should be adding more actual acid? It seems like the original acid solution that existed when I bought the batteries would be overly diluted at this point.

You should be checking specific gravity 24 hours after you add water and 2+ hours after fully charging to assess the health of the electrolyte. If your specific gravity is low, you should conduct an equalization charge.

When you're charging a lead acid battery and its gassing off, it is gassing off the hydrogen and oxygen out of the water, the amount of sulfric acid molecules should remain the same.
