Interesting BMS behavior: is this normal?

If your batteries regularly see > 3.45V/cell, then hopefully, that should be sufficient for balancing.

Equalization in the traditional sense is exclusively for flooded lead acid batteries.

With the 48V equalizers linked above (or a similar product), that may be sufficient to never have additional concern or need effort for maintaining balance.

They do hit that voltage for a bit, but I don't know if that's long enough, as I followed your advice to set the float to 54V, which they seem to be rather happy with, but, there's definitely no balancing happening at that voltage.
