Combiner Boxes-when to use. I have 4 ground mounts each holds 4 panels for an overall 16 panels. Do I need a combiner box?

Well more than 2 in parallel needs fuses. You should have disconnects. Blocking diodes are usually useful. The connection point saves wire versus homeruns.
So probably a 4-2 or 4-1 makes sense

Either a combiner box or a handful of inline fuses. The combiner box is the much better option.
